How to get a copy of your GP record

Health records contain information about your health and any care or treatment you’ve received.

Your health records may contain:

  • test and scan results
  • X-rays
  • doctors notes
  • letters to and from NHS staff

It’s important that your records are kept up to date. You should tell NHS staff when your personal information changes or if you are going to be out of the UK for a long time.

You can request a copy of your records under the GDPR (and DPA 2018) is called a Subject Access Request (SAR).  If you want to see your health records, or wish a copy, please complete a Practice Subject Access Request Form which you can get at Reception.

Accessing someone else's information

Consent can be given for us to share your clinical information with a specified person on your behalf with your written permission.

This may be neccessary if a person is unable to speak for themselves due to a medical condition they may have. 

Please ask our Reception staff for the suitable form if this is somthing you would like to do or download the form.

Please note that it is important that you inform the practice immediately if your consent contact details change or you wish the person given consent to be removed from your record.

Remove or update details of who can discuss my medical records

We understand that situations change. If you need to update details or remove access for an individual being able to dicuss your medical records please ask our Reception staff for the suitable form or download the form.
