Chronic Disease Clinics

If you suffer from one or more chronic disease i.e. asthma, COPD, diabetes, Chronic Kidney disease, coronary heart disease or have had a stroke, you will be invited to attend for a health review with one of our Practice Nurses. This will involve a 20-40 minute appointment or telephone consultation. You may also be given an appointment to have blood tests prior to the health review so that the nurse has your results to discuss with you at your appointment.
